• بيان تأكيد بأنك تعتقد بحسن نية أن استخدام المادة غير مصرح به.

• بيان بأن المعلومات الواردة في الإشعار صحيحة، وتحت طائلة الحنث باليمين، أنك صاحب الحقوق أو مفوض قانونيًا بالتصرف فيها.
